The Voice September 2002 The UUP Benefit Trust Fund has moved. While our main toll-free number has remained the same, we have new fax numbers and a new address: UUP Benefit Trust Fund, P.O. Box 15143, Albany, N.Y. 12212-5143. The phone numbers are (518) 640-6680 and toll-free at (800) 887-3863. The fax numbers are (518) 640-6699 and toll-free at (866) 559-0516.
When you call the UUP Benefit Trust Fund, the phone will be answered by one of the UUP Benefit Trust Fund representatives: Jan Peters, Sue Tocco, Joyce Montgomery or Sharon Houle.
If all Fund representatives are on their lines, the call will be forwarded to Liz Kahnle, associate director of Member Benefits and Services, or Candy Keyoskey, benefits assistant. If they are also on their phones, the call will go into voice mail.
When leaving a voice-mail message, please include as much detail as possible, as well as identifying information and a number where you can be reached. The UUP Benefit Trust Fund policy is that all phone calls received on the Fund line by 4 p.m. will be returned the same day.
Student verification
Now is the time students return to college. To the Fund, that signifies time for student verification. Members who have dependent children between ages 19 and 25 who are full-time students are eligible for UUP Benefit Trust Fund coverage. For a child over age 19 to continue benefits, we must have a completed student proof form in our office. Student proof forms were recently mailed to members with children in this age category. If you need a form for your child, call the Fund or print one from our Web site at www.uupinfo.org. Click on benefits on the left bar; choose benefits forms, then student verification form.
To complete this form, you will need college information for your dependent child and a notary signature. The form can be mailed or faxed to the Fund.
Please note that this information is not shared between the Fund and your health insurance carrier. You must send separate student verification to your health carrier.
